Step 1: Phone Consultation

After contacting Dr. Renken, a phone consult will be scheduled to speak about your concerns and what type of evaluation would be recommended. After the consultation is complete, a cost estimate will be sent via email. If interested in pursuing the evaluation, intake paperwork will be sent to be completed before the initial intake appointment.

Step 2: Intake Interview

An intake interview will be conducted (in-person, phone, or virtual conference) in order to obtain more details about your concerns. Prior to the intake interview, the following should be provided:

  1. Relevant psychological, medical, or school records (e.g. school evaluations, counseling/therapy notes, previous emotional/behavioral assessments)
  2. Contact information for teachers, physicians, or mental health providers
  3. Intake forms

After the interview is complete, the service contract, payment options, and other policies will be reviewed. An evaluation date will be scheduled as soon as a signed contract is returned.

Step 3: Testing/Evaluation

Depending on the type of evaluation, testing one-on-one with the client can last anywhere from a few hours to a full day with provided breaks. Depending on the concern and tolerance of the child, sessions across multiple days may be needed. Based on the referral concern, the evaluation may consist of clinical interviews, standardized tests (e.g. cognitive, academic, or executive functioning tests), computerized assessments, rating scales, or behavioral observations. Outside of one-on-one testing, other evaluation measures such as interview with parents, teachers, physicians, or other professionals may occur. Depending on the referral concern, rating scales may be sent out to parents and/or teachers.

Feedback

After testing has been completed and information from all sources has been gathered, a feedback session will be scheduled within 2-3 weeks to review the results the evaluation. A written report will be provided after the feedback session. The report will list all test results, interpretations, diagnoses as appropriate, and recommendations regarding possible accommodations or special education eligibility (if applicable). Copies of the written report will not be shared anyone other than the parent/guardian unless they specifically authorize its release (see Policies page for more information regarding confidentiality and records management).
